The Terms & Conditions of the member agreement may change from time to time & we will update the same on the website at least 2 working days in advance. You are hence advised to refer to the website on a regular basis so that you are aware of the latest applicable member terms & conditions.

Consultation

We invite & value feedback from you about the website and, from time to time, we use the website to carry out electronic consultation exercises. If you send comments to us via the website, we will collate your comments and your contact details for the purposes of the consultation only.

Third Party Service Providers

We have contracts with other organizations (or Third Parties) to carry out certain activities or services on our behalf. If we need to pass any of your personal data to them, we will ensure the following:

They are only provided with the information they need to carry out the service.

They agree not to use the information received from us for any other purpose than those specified by us.

Registration of Medical Practitioners

If you provide your personal & contact details when inquiring about registration with Allied Health Organisation or otherwise, we will use that information for the following purposes:

• To process your enquiry or application

• compile statistics for our internal purposes

• To send you relevant documents and our newsletter

We may supply information to the Department of Health, Professional, Corporate, Education and Training Bodies so they can compile statistics

We may retain details of your application on our files and may disclose that information to overseas regulators and elsewhere if required. If you are subsequently empanelled/ registered with Allied Health Organisation, we will use the information you provide us with for the purposes listed below:

• Updation of our database

To share your contact details with members, corporates & institutions who wish to access your services Administer and maintain your registration details Process complaints, if any To maintain and compile statistics for internal use Keep you updated with information about the Allied Health Organisation including guidance, news, etc.

Allied Health Organisation Members

If you are registered with Allied Health Organisation or are aAllied Health Organisation member, we will use the information you provide us with for the purposes listed below:

• Updation of our database

• Administer and maintain your registration details

To maintain and compile health records including Thyrocare/ DNA test reports or any health records and results for internal use To maintain it for analytical purposes including to provide necessary consultation/suggestions, if required based on the health reports of the Allied Health Organisation members

Covid19 Plans Utilization Ability

Claim Admissibility Criteria: Member should be tested positive for the below listed disease with confirmation by a qualified medical practitioner to be eligible for claim under the policy.

Disclosure

We may be asked to provide your contact details to the MCI, DCI, IMA, IDA, medical defence organizations, medical colleges, faculties & other organizations so that they can keep their records up to date. If you would prefer us not to share your registered address with these organizations please let us know by writing In addition to this, we may disclose your personal information; to the extent that we are required to do so by law;

• in connection with any ongoing or prospective legal proceedings;

• in order to establish, exercise or defend our legal proceedings;

• in order to establish, exercise or defend our legal rights (including providing information to others for the purposes of fraud prevention and reducing credit risk); and to any person who we reasonably believe may apply to a court or other competent authority for disclosure of that personal information where, in our reasonable opinion, such court or authority would be reasonably likely to order disclosure of that personal information.

Grievance Redressal Officer

In case Allied Health Organisation members have any grievances in connection with the use of the website, a member may write to the grievance redress officer at [email protected] or [email protected] who is authorized to attend to any user complaints or queries, and may resolve any such matter within 1 (one) month from date of receipt of complaint.

As an extra precaution your session on many areas of our website will terminate if you have not used the site for 15 minutes. discontinue the Preventive Health Care Plan issued to you and your membership with Allied Health Organisation with immediate effect; and issue a refund of the amount paid by you as Membership Fees.

Cancellations shall be considered only if (i) a valid request is made in this regard within 30 (Thirty) days of receipt of the Preventive Health Care Plan by you, and (ii) the entire Preventive Health Care Plan Welcome Pack is returned to Allied Health Organisation within 14 (fourteen) days of receipt of the Cancellation request as aforesaid. For calculation of the aforesaid periods, the records of dispatch, delivery and communications available with Allied Health Organisation shall be considered final.

No Cancellation shall be carried out if you have purchased the Preventive Health Care Plan under any celebratory / festive offer, or any special / discounted offer of any kind whatsoever.

No Cancellation shall be carried out if you have purchased the Preventive Health Care Plan under any interest free easy monthly instalment (EMI) option offered by Allied Health Organisation.

No Cancellation shall be carried out if you have in any way availed any of the benefits available under the Preventive Health Care Plan prior to confirmation of Cancellation by Allied Health Organisation.

In the event a request for Cancellation is received by Allied Health Organisation after dispatch of the Preventive Health Care Plan Welcome Pack to you, Allied Health Organisation Medi Health LLP shall deduct an amount of Rs. 1,000 (Rupees one thousand only) or 10% of the membership fee whichever is higher from your Membership Fees and only the balance shall be refunded to you as part of the Cancellation.

In the event a request for Cancellation is received by Allied Health Organisation after a positive welcome call to you, Allied Health Organisation shall refund up to a maximum of 50% of the membership fee of your Membership Fees only.

Processing fee charged under the ‘EMI Option on Credit Cards’ program is non-refundable.

All Cancellations as approved by the Allied Health Organisation management as per their discretion shall ordinarily be processed within 21 days of receipt by Allied Health Organisation of the Preventive Health Care Plan Welcome Pack. In the event the entire Preventive Health Care Plan Welcome Pack is not received by Allied Health Organisation within 14 (fourteen) days of receipt of a valid request for Cancellation by Allied Health Organisation, shall deduct an amount of Rs. 2,500/- (Rupees two thousand five hundred only) or up to 50% of the membership fee whichever is higher from your Membership Fees and only the balance shall be refunded to you as part of the Cancellation.

Credit of the Membership Fees to your bank account / credit card account upon Cancellation and refund by Allied Health Organisation shall be as per policies and timelines followed by the banks involved and Allied Health Organisation shall not be responsible for any delays in this regard.

All Bills to be sent within 30 days of procuring them from various Medical Practitioners.

Disclaimer

Allied Health Organisation products are not insurance products or insurance offerings. Allied Health Organisation offers Preventive Healthcare Checkup plans at concessional/Subsidised rates. In addition, Allied Health Organisation also offers Concessional/Subsidised Check-Ups & Treatments across Allied Health Organisation certified Doctors, Laboratories, Dentists, Pharmacies, Hospitals, Nursing Homes, Medical Centers. Allied Health Organisation, Healthcare Plan member can claim deduction to the extent of Rs. 5000/- for Preventive Health Check-up under the amendment to section 80D of the Income Tax Act, 1961 in the Finance Bill 2012. The benefit of Section 80D is over and above the limit of Rs. 1,50,000/- prescribed under Section 80C/80CCC. This benefit is available for expenditure incurred towards Preventive Health Check-up of Self, Spouse, dependent Children and Parents of the member.
